As a guest I would hate to be asked *my* preferences for a meal. I want
to be the guest, not the center of attention or dictator. I would feel
as if I were organizing their hosted meal. Awkward!
I would just offer a well rounded meal and assume they'll have enough to
choose from to suit themselves.
If the guest has such bizarre dietary requirements that it would be
difficult to feed him, I'm sure he'd have let them know when accepting
the invitation in the first place?

As for Jill and the dessert issue- special meals are often made more
special with the inclusion of desserts. This has nothing to do with how
you were raised to not have them or how fat you are. It has to do with
celebrating the special meal with something out of the ordinary.
