My bifocals for a long time had a distance prescription in the top and
zero diopter in the bottom--I had 'em because I was teaching and
couldn't see the back of the class clearly without the distance lenses
but had to work at seeing my notes with 'em.

Order 'em off Zennioptical--they'll make anything you ask for and don't
cost much more than the ones on the rack. My reading is now .5 and it's
a matter of being able to see everything on my desk with 'em or having
to back off a bit to see close stuff clearly without 'em.
