They're usually with someone who can see, that person can do the
ushering if it's absolutely necessary.

Honestly, I think you're over reacting. I live in a very handicapped
accessible area and yet I'm having trouble remembering when I've seen
a guide dog inside a restaurant that wasn't fast food. I think when
blind people dine in a white tablecloth restaurant, they are usually
with a seeing person who acts as their guide.
