If your using a cooking spoon and sieve, those things are going to resist
being rubbed through and you can easity sort them back out. We use a food
mill. The larger bits all wind up on top of the blade but a significant
portion of the veggies do go through.

Now, if you had dropped this in a blender, everything would be pureed.

One would expect a person doing like this to exercise a bit of sense.
Still, yours is a good point and I need to stop assuming that people will
do the reasonable thing.

Perhaps I should indicate the use of a food mill in that recipe.
