From what I understand, the process can be quite painful as the cells
stop getting sufficient oxygen. There skin's appearance changes and it
gets blotchy. Doctors and nurses can often tell how imminent death is by
the appearance and the rate of change. It's is not always a great idea
to hold a person's hand close to the end because the physical contact
can be quite painful.
