It's a shame that anyone should have to call in OSHA to enforce existing
regulations or company safety rules. It's not like inspectors will be
there immediately, or that they can complain without fear of repercussions.

A number of years ago there was an accident at my neighbour's place. He
was having his driveway repaved. The owner of the company was trying to
clear dirt out of the box of a dump truck. The dump box was up, tail
gate open, and he was poking around with a shovel..... while the back
hoe tried to loosen the dirt.

I was mowing my lawn and spotted it and was shocked at the accident
waiting to happen... and it did. The book hit the tailgate and it
squeezed the guy's head. I did the first aid on the guy. Ambulance,
police showed up. Police called the Ministry of Labour and they sent an
inspector out. All of a sudden all the workers were wearing hard hats.

MoL did their investigation and left. As soon as the inspector's car
left the hard hats came off.
