You are NOT required to give your SSN to your doctor, dentist or
anyone else who has no direct legal need for such information. Doctors
and dentists' offices, for instance, routinely ask for your SSN. They
have no legal right to this information. I have only been turned away
once (a radiology clinic) when I refused to give them my SSN and I
reported them to the state commission that oversees such entities. Got
a mondo letter of apolgoy from the radiology clinic a few months
later. If you have any questions as to whether an entity is entitled
to your SSN information, don't give it to them until you are better
informed as to why they need/are entitled to it.
