Default Nightmare at BENIHANA'S in Santa Monica, CA

That was an unhappy story, but not quite a teppan nightmare.

A local sushi/teppan place actually had the nightmare.

While making a flaming shrimp volcano, something went wrong with the
fuel and it exploded, showering two young girls (4 and 5, iirc) with
flaming liquid.

It's a sad situation, especially for those girls and their family, but
somewhat for the restaurant, as it was almost certainly accidental
rather than incompetence, but the restaurant no longer does teppanyaki,
and may never get enough insurance to start it up again. It's still
one of the most popular sushi restaurants in town, though, so it may
survive, but it can't be running a profit right now with all that
unusable space and depreciating equipement...
