No answers, just two observations. One, that it looks like some kind of
early obstetrical device; two that it is about the shape of an avocado.

Now that you mention it, wasn't the avocado known at one time as the

I thought it might be a nutcracker shortly before
seeing the first reply in the thread. Though
I was thinking maybe a cracker for the large
round pods that hold Brazil nuts (seldom seen
in the U.S.). This idea seemed a bit too
unlikely, but the general idea of a nutcracker
seems reasonable because the bowl of the spoon
would catch the bits of meat and shell.

However, the screw action would be a slow and
tedious way to shell nuts. I'm now thinking
maybe it's for shelling black walnuts. They
are hard little guys, much more difficult to
crack that regular walnuts, and their nearly
spherical shape causes them to slip out of
many other types of nutcracker. The slightly
cupped end on the screw looks like it would
be just right to hold a black walnut. The
tedious screw action might be acceptable because
nobody uses a large number of black walnuts.
They are usually used in small amounts in
candy, cakes, and ice cream.
