It's actually not bizarre, and really quite common. Here in Oz, we are very
particular about what species of fish we eat. I've been eating fish since I
was about 10yo, and have never had it. We were schooled on Ciguatera even
back then.

The cold allodynia (temp reversal) suffered by the woman shows that she had a
severe case of the poisoning. That's common in severe cases as well.

Sort of shock to me. We've eaten grouper and other predator fish for
years. The only ones we wouldn't eat were barracuda as the ciguatera was
well known in those, particularly the large, solitary cudas.

Guess we've been lucky over the last seventy years.

Admitedly, I've always lived near, interacted with, the ocean.

I've always been at least a long day's drive or more from either US coast.
The best I can say is that I grew up on the shores of Lake Michigan -- a
larger body of water 'o} There may be large fish there, but I don't think
any of the size a grouper can attain.

Ciguatera is scary stuff. I've known about it ever since reading about it in
a Reader's Digest article some twenty years ago. The toxin is not killed by
cooking, so fully-cooked fish are no safer from ciguatera than sashimi.

Whenever someone tried to argue there are no health benefits
to being vegetarian, I point them to information such as this...
