#1 Sounds like a Crate & Barrel type item. I "think" that's where my
mom purchased her two tiered plate holder but it was a couple of years
ago and they change stock mighty quickly.

On to #2 I'm an antique dealer by trade and used to have a pattern
replacement service. Email me off list with what your backstamps in
question read and I'll try to give you a hand. If you have webspace to
post photos to, I or the folks on the Ebay pottery/china boards may be
able to help you with some pattern IDs.

I've noticed that Sam's Club is stocking a fair amount of buffet
servingware, starting with use-and-toss aluminum chafing dishes and on up.
