I am interested in formulating a critique sheet for the people who I give
bottles of wine. They seem to graciously accept the wine and offer
feedback. Most ask specifically what information I want, so to be
consistent, can anyone offer a evaluation form or questions to help with the
evaluation of my wines?

Only you know what feedback you want. As for me, I just wrote myself a
really '**** in my own pocket' e-mail and send it to myself every evening so
I get something worthwhile to read in the morning.
